چکیده

Introduction: Brown tumours are boney complications of hyperparathyroidism. They giant cell lesions that often appear as an expansile osteolytic lesion of the bone. We discuss the diagnosis and management of a brown tumour that presented as a non-healing extraction socket. Case Report: A 59 year old male presented with a one year history of a non-healing extraction socket on the lower left mandible following the removal of the LL6. The socket was not associated with any acute symptoms such as pain or swelling, but lack of mucosal closure persisted despite local measures with his general dental practitioner. The patient had a medical background of hypercholestrolaemia, hypertension, and anxiety. He previously had hyperparathyroidism as a result of a parathyroid adenoma, which was surgically removed 6 months after the dental extraction. Intra-oral examination revealed a suspicious-looking heterogenous lesion on the lower left alveolar ridge within the extraction socket. Radiographically there was a diffuse area of irregular radiolucency along the lower left mandible which had a moth eaten appearance in the absence of retained tooth fragments. An urgent incisional biopsy was undertaken which showed giant cells present. Bloods revealed mildly elevated calcium levels and low levels of phosphate. A diagnosis of Brown’s Tumour was made and the area was curetted and treated with Leukocyte-Platelet Rich Fibrin (L-PRF) and primary closure. Discussion: We discuss the possible differential diagnoses of non-healing tooth sockets together with the pathology of Giant Cell Lesions and their surgical management including the benefits of using L-PRF to improve healing. Conclusion: Brown tumours in the mandible can present as a non-healing extraction socket. Curettage and treatment with L-PRF is enables boney healing and is suitable management technique for this type of presentation.

The Relationship between Socket Blood Sugar and Post-Extraction Complications in Type II Diabetic and Non-Diabetic Patients

Abstract Objective: Prevalence of tooth loss is higher among diabetic patients in comparison with non-diabetic patients. The aim of this study was to evaluate and compare the relationship between socket blood sugar and post-extraction complications in type II diabetic and non-diabetic patients. Materials and Methods: This cross-sectional study was carried out on 80 diabetic and 80 non-dia...

The Effect of Calcium Alginate Dressing on Bleeding and Complications After Tooth Extraction

Abstract Background and Aim: Post extraction Bleeding is a well-known and frequent complication that can continue for 8-12 hours after surgery and if not controlled, it can create a range of complications from a simple hematoma to a large amount of blood loss. Calcium alginate dressing, can result in  hemostasis and accelerate healing of tissue. The aim of this study was to determine the effec...

Effects of local and whole body irradiation on the appearance of osteoblasts during wound healing in tooth extraction sockets in rats.

Irradiation before tooth extraction delays wound healing in the alveolar socket. This study examined the influences of local and whole body irradiation before tooth extraction on appearance of osteoblasts in the alveolar bone of rat maxillary first molars because bone formation is observed at the initial phase of wound healing. Several osteoblasts were generated 3 days after tooth extraction, a...

Primary intraosseous squamous cell carcinoma of mandible: a case report

Background: Primary intraosseous squamous cell carcinoma (PIOC) of the mouth is a very rare but well-known carcinoma. It is locally invasive and its prognosis is quite poor. It may originate from the walls of an odontogenic cyst or de novo from the remnant epithelium at this region. Because the early symptoms of such malignancies are sometimes similar to those of inflammatory or periodontal dis...

Influence of different types of non-resorbable suture material on the healing of extraction wounds--a histological study in rats.

The influence of different non-resorbable suture materials on the healing of tooth extraction wounds was examined in 120 adult male albino rats. Nylon gave the best biological results for healing of the mucosa and dental socket. Silk and cotton produced considerable delay of socket healing. Healing was only mildly delayed by polyester suture, with a short moderate inflammatory reaction at the i...
